The interoperability argument is the one that's been underappreciated for years. The fundamental misalignment: sites want consistent workflows across sponsors, while sponsors want sites to adopt their proprietary systems. This is a textbook collective action problem that markets won't solve on their own. Tying certification to ONC makes sense given its existing health IT infrastructure expertise, but I'm curious how the FDA and ONC would reconcile their different priorities—FDA is primarily concerned with data integrity and patient safety, while ONC has historically focused on clinical care interoperability. Bridging that governance gap would be one of the hardest parts.
